PAR-Q FORM

More people are starting to realise how good exercise is for your mental health, your appearance and how fun it can be. Increasing your physical activity levels can be very safe for the majority of people. But, for some, it may be a better idea to check in with your doctor first. 

If you are planning to start doing more exercise than you are doing at this current time, start by answering the following seven questions. If you're between the ages of 15-59, this PAR-Q will help you decide whether a discussion needs to be had with a doctor before making arrangements for further physical activities. 

Please answer each question with honesty, it is for your safety after all. 

If the answer was YES to one or more of these questions -

  • Please speak to your doctor before you start to increase any physical activity, make them aware of the questions you answered yes to and see if there are any compromises etc.

  • There may be a chance of increasing exercise levels as long as you take it slow in building it up gradually or even restrict yourself to the activities you know you are safe to do. 

 

If you answered NO HONESTLY to all of the questions, you can rest assured that you are -

  • Safe to become more active, begin slow and build your stamina up. 

  • A great way to establish your fitness level is to take part in a fitness appraisal. It's recommended that you take a blood test and if it reads over 144/94, you will need to speak to a doctor. 

 

Delay becoming more active if -

  • You have a temporary illness, such as a cold or a fever. Wait until you are better.

  • You may be pregnant, please speak to a doctor before increasing exercise levels.

 

IF YOUR HEALTH CHANGES TO YES AT ANY TIME, PLEASE INFORM YOUR FITNESS OR HEALTH PROFESSIONAL.
